All PPO Plans Accepted
We accept all PPO dental insurance plans, including plans from the commonly accepted carriers below. Network participation and benefits can vary, so call with your member information and we’ll verify your specific coverage.
Plans we do not accept: Medicaid, AHCCCS, HMO, or DMO. If you have one of these plans—or do not have dental insurance—our in-house Dental Loyalty Plans in the next section offer another way to access the care you need or want.

The loyalty plan is not insurance. Membership runs 12 months and renews automatically; you may cancel for a full refund (less processing fees) within 30 days of your start date. Benefits are usable only at Toothology, do not carry over, and cannot be combined with insurance, other discounts, or special offers. Review the enrollment page or ask the team for current terms.

Will I Know My Cost Before Treatment?
Yes. Once the dentist has recommended care, our team reviews expected out-of-pocket costs with you before anything is scheduled. Insurance estimates are our best projection of your benefits, not a guarantee of final payment.
Is the Loyalty Plan Insurance?
No. It is an in-house membership plan with included preventive visits and a 20% discount on procedures at Toothology. There are no deductibles, annual maximums, or waiting periods.
Can I Combine the Loyalty Plan With Insurance?
Plan benefits cannot be combined with insurance, other discounts, or special offers. If you have dental insurance, we will help you compare which option saves you more.
